Doxorubicin Dose Deintensification in Pediatric Osteosarcoma, Is Less Better?

چکیده

Introduction We implemented new clinical practice guidelines (CPG) for patients with osteosarcoma starting in January 2009. These were based on standard European and American Osteosarcoma Study regimen, which includes six cycles of doxorubicin a cumulative dose 450 mg/m2. Aiming to reduce cardiac toxicity at our center, we opted the 375 Methods This is retrospective cohort aged <18 years, treated center between 2009 2018. Patients unified CPG prospectively followed. Disease treatment characteristics depicted, survival rates calculated. When needed, comparison different groups conducted using log-rank test. Results After median follow-up 43.3 months (range, 2–153 months), 79 diagnosed dose-reduced doxorubicin. Median age diagnosis was 12.8 years. At diagnosis, 58 (73%) had localized disease. The 5-year event-free (EFS) whole group 50 ± 5.9%, overall (OS) 64 5.7%. For extremity nonmetastatic tumors (N = 56), EFS OS 60 6.9% 70 6.8%, respectively, this patients, response chemotherapy associated better (p 0.0048) 0.013). Only two suffered transient dysfunction, resolved after treatment. Conclusion Our findings suggest that deintensification may provide adequate control pediatric osteosarcoma. In absence large randomized trials addressing issue, developing countries less resources treat heart failure consider lower dose.

Oxygen – less is better , more is better ? ?

Altitude training has been employed as a tool for enhancing performance by athletes since the 1964 Mexico Olympics. The cost and impracticality of going to altitude regularly has led to the development of altitude simulation at sea level by creating low oxygen (‘hypoxia’) environments. ‘Altitude’ or ‘hypoxic’ training facilities are now springing up all around the country. The evidence supporti...

Matrix metalloproteinase-activated doxorubicin prodrugs inhibit HT1080 xenograft growth better than doxorubicin with less toxicity.

Matrix metalloproteinase (MMP)-activated prodrugs were formed by coupling MMP-cleavable peptides to doxorubicin. The resulting conjugates were excellent in vitro substrates for MMP-2, -9, and -14. HT1080, a fibrosarcoma cell line, was used as a model system to test these prodrugs because these cells, like tumor stromal fibroblasts, expressed several MMPs. In cultured HT1080 cells, simple MMP-cl...

Bromide interference: is less really better?

Increased serum bromide may result from the ingestion of various drugs and from environmental exposures. Chloride methods that are less susceptible to bromide interference have a clinical disadvantage in that they are less likely to arouse suspicion in cases of bromide toxicity. When there is a clinical suspicion of bromide toxicity, differences among various analyzers in the amount of interfer...

Plant Breeding: Surprisingly, Less Sex Is Better

Introduction of apomixis, asexual reproduction through seeds, into crop species has the potential to dramatically transform plant breeding. A new study demonstrates that traits can be stably transferred between generations in newly produced apomictic lines, and heralds a breeding revolution needed to increase food production for the growing planet.
